GP Patient Survey - latest results
You are welcome to attend at either Cranleigh Gardens in Bridgwater or at our branch surgery in Westonzoyland. Full details of how to register are available at either surgery. Please contact reception where you will be asked to fill in a registration form or bring in your NHS medical card and some form of identification that includes your address.
We would like to obtain your medical history as soon as possible so you will be asked to complete a medical questionnaire. This will enable us to provide the best possible medical care for you and your family.
If you wish to register, please call in to the surgery to collect a registration pack or, if you prefer, complete the forms below, print at home and drop them into the surgery.
Please note that completing these forms does not guarantee that you will be accepted as a patient, please talk to the Receptionist when you attend.
We will need to see identification, including proof of address when you register.
Registration Form When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.

If you are visiting from another country and need urgent medical attention we will treat you under the NHS system for a period of up to two weeks.
Please note that hospital care or other community care may incur a charge, depending on your country of residence.
Further information on who is eligible for NHS treatment and what charges might apply is available via NHS Choices
If you are moving abroad on a permanent basis, you will no longer be entitled to medical treatment under normal NHS rules. This is because the NHS is a residence-based healthcare system. Please notify us so that you and your family can be removed from the NHS register.
Most people will no longer be entitled to use your UK-issued European Health Insurance Card (EHIC) to access healthcare abroad.
